The Office of the Chief Coroner called for stricter fire inspection rules and a system that would “red flag” any property that has a history of violating fire codes after two deadly house fires claimed the lives of three teenagers and a family of four.

The inquest examined everything from the fire alarms in the home to the training of the 911 operators who were alerted about the fires.

Both fires were deemed to be accidents, but the coroner launched an inquest into the fires in hopes of learning valuable lessons for the future, which resulted in 33 recommendations.
